1. Purpose & Scope

The Life Changer ("we", "our", "us") respects your personal privacy. This policy describes how we collect, process, and protect your information when you interact with our website and complete our guided wellness intake form.

2. Personal Data We Collect

To respond to your programme enquiry and understand your baseline lifestyle habits, we collect:

  • Identity & Contact: Full name, email address, and optional phone number.
  • Demographics: Age in years (restricted to adults aged 18–100).
  • Physical Baseline: Self-reported height, weight, and general weight-management aspirations.
  • Dietary Information: Self-reported food allergies, dietary restrictions, food avoidances, and meal frequency.
  • Fitness & Activity: General exercise background, session time availability, and physical movement limitations.

3. Sensitive Health-Related Information & Explicit Consent

Our intake form includes an optional health information step allowing individuals interested in our diabetes lifestyle education to share self-reported diabetes status or blood glucose measurements.

Explicit Consent Requirement: We do not collect or transmit health-related data without your separate, affirmative checkbox consent. If you do not provide this consent, you can continue with a general enquiry and any entered health readings are excluded.

4. Integration & Processing Architecture

Form submissions are securely sent via HTTPS to our configured workflow system (Pabbly Connect). We do not send your health data to advertising networks or tracking pixels.

5. Data Retention & Your Rights

You retain the right to request access to your submitted data, request corrections, or ask for complete deletion of your records from our intake system. To exercise these rights, please contact us with your submission reference number.

6. Health & Medical Disclaimer

The Life Changer provides general wellness education. It does not replace individual medical advice, diagnosis, or treatment. Consult a qualified healthcare professional before making significant changes to diet, exercise, or diabetes management.
